Participation Prerequisites
All full-time employees, regular part-time employees, regional employees, employees of SAS subsidiaries, summer students and spouses/registered domestic partners and dependent children over the age of 18 living in the same household may use the RFC facilities at any time.
They are also eligible to participate in any "free play" activities at the RFC, including non-league sports, and to use the RFC locker rooms.
Forms required for participation in all other RFC activities are listed below. In all cases, return completed forms to Building G.
Use of Cardiovascular Equipment
- Either: A health check from the SAS Health Care Center.
- Or: Medical Evaluation form(PDF) completed by your primary care provider.
- And: Consent and Release form(PDF).
Use of Nautilus Equipment and/or Free Weights
A Nautilus/free weight orientation with an RFC Trainer is required prior to using this equipment. A trainer will contact you to schedule this appointment once your request is received and your Medical Evaluation form is on file at the RFC.
- Either: A health check from the SAS Health Care Center.
- Or: Medical Evaluation form(PDF) completed by your primary care provider.
- And: Consent and Release form(PDF).
Participation in Aerobics, Yoga and/or Tai Chi Classes
- Either: A health check from the SAS Health Care Center.
- Or: Medical Evaluation form(PDF) completed by your primary care provider.
- And: Consent and Release form(PDF).
Participation in a Fitness Program or Sports League; Wellness Profile
- Either: A health check from the SAS Health Care Center.
- Or: Medical Evaluation form(PDF) completed by your primary care provider.
- And: Consent and Release form(PDF).
Use of the Pool and/or Participation in an Aquatics Class or Sports League
- Either: A health check from the SAS Health Care Center.
- Or: Medical Evaluation form(PDF) completed by your primary care provider.
- And: Consent and Release form(PDF).
- And: Aquatics Policy form (available from the RFC).
